Instructions for submitting forms : (submit form area are underneath these instructions)

1: Download and save the appropriate form for what you are sending.

2: Fill in all required fields. Fields with an asteric (*) are required fields. Incomplete chain of custody could result in testing delays.

3: Save a copy of the completed chain of custody for your records. (Either hard copy or electronically)

4: Fill in the email address, name and phone number of whoever is submitting test request. (may be different than person authorizing payment).

5:  Upload the document(s) that you wish to send.

6: Using your mouse as a pen, electronically sign in “Signature” box.

7: Hit "Submit" to send to Vallid Labs.
